General and Durable Power of Attorney 
Fee: $150.00

 

A Power of Attorney allows you (the principal) to authorize another person (the agent) to handle financial, legal, and property matters on your behalf. A General Power of Attorney is effective immediately and grants broad authority, while a Durable Power of Attorney remains effective even if you become incapacitated. A Springing Power of Attorney becomes effective only upon the occurrence of a specific event, such as a physician’s determination of incapacity. We can prepare any of these forms based on your needs. The $150 flat fee includes preparation of the Power of Attorney, notary services for the principal, and certified copies as needed. This document is not filed with the court and is used privately as needed. This service is limited to document preparation and execution formalities and does not include legal advice.

Medical Power of Attorney 
Fee: $100.00

A Medical Power of Attorney allows you to appoint a trusted person to make health care decisions on your behalf if you are unable to communicate your wishes. This document may include authority over medical treatment, health care providers, and end-of-life decisions, and works in conjunction with (or separately from) a Living Will or Advance Directive. The $100 flat fee includes preparation of the Medical Power of Attorney, notary services, and certified copies as needed. This document is not filed with the court and is intended to be used by medical providers when needed. This service is limited to document preparation and execution formalities and does not include legal advice.
