Adult Name Change
Fee: $600.00

 

An Adult Name Change is a court process used to legally change an adult’s name. The $600 flat fee includes preparation of the Civil Cover Sheet, Petition for Change of Name, Notice of Publication, Affidavit of Publication, Request for Submission, and Proposed Order, along with notary services, required court copies, and filing with the court. Court filing fees are not included and are paid separately; in the Fourth Judicial District (Elko County), the current filing fee is approximately $275, though fees may vary by court. Newspaper publication is required for adult name changes, and newspaper fees are not included and typically range from $80 to $120, depending on the publication. This service is limited to document preparation and filing and does not include legal advice or court representation.

Child Name Change
Fee: $600.00
A Child Name Change (By Agreement) is used when both parents consent to changing a minor child’s legal name. The $600 flat fee includes preparation of the Petition for Change of Name, Request for Submission, and Proposed Order, along with notary services, required court copies, and filing with the court. Court filing fees are not included and are paid separately; in the Fourth Judicial District (Elko County), the current filing fee is approximately $275, though fees may vary by court. If one parent does not agree to the name change, a Complaint for Change of a Child’s Name must be filed instead; please contact DPS of Northern Nevada for additional information regarding that process. This service is limited to document preparation and filing and does not include legal advice or court representation.
